How they work

Print it, check it, make it yours. Each template is a starting point to check against current guidance and your own policies, never a sign-off.

  • Prints on its own

Print or save as PDF and only the template comes out, sized for A4 and ready to fill in by hand.

  • Sources on every page

Each template lists the official guidance and legislation it was checked against on 26 October 2026, and anything we couldn’t confirm.

  • Yours to adapt

Add your organisation’s name, escalation contacts and policy references before your team uses it.

Or keep the record where the work happens. In Diversity Sync’d the NDIS Incident Report, Child Safety Incident Report and Behaviour Record come ready to use. Workers complete them inside the shift note from their phone’s browser, and every submitted form is ready to export as a PDF.

  • Shift notes on one timeline per participant, with a reminder before a note goes missing
  • An NDIS register with the 24-hour, 5-day and final report dates for each incident
  • Queensland reportable conduct cases with 3- and 30-business-day clocks
  • Your own forms, built by drag and drop with photos and drawn signatures

FAQ

Questions, answered. About the templates, and what to do with them.

No. Each one is a starting point we checked against official guidance and legislation, with every source listed on its page alongside anything we couldn’t confirm. Rules, portals and forms change, so check a template against current guidance and your own policies before you rely on it.

No. Open a template, select Print or save as PDF, and print it or save it from your browser’s print window. There’s nothing to sign up for.

Select Print or save as PDF on the template’s page, then choose Save as PDF as the destination in the print window. Only the template prints, without the rest of the page, sized for A4.

No. Reportable incidents are still lodged in the NDIS Commission Portal, and reportable conduct reports go to the QFCC through its online portal or on its paper forms. The templates help you gather what those reports ask for while the facts are fresh.

Possibly both. In Queensland, an allegation about a worker’s conduct towards a child can be reportable conduct for the QFCC, and if the child is an NDIS participant it may also be a reportable incident for the NDIS Commission. Each has its own clock: 24 hours for most reportable incidents, and 3 business days for the QFCC’s initial report. If you’re unsure, ask the regulators.

Yes. The NDIS Incident Report, Child Safety Incident Report and Behaviour Record come ready to use and are filled in inside the shift note, workers write shift notes on one timeline per participant from their phone’s browser, and Queensland reportable conduct cases run with 3- and 30-business-day clocks. Form submissions and collaborative shift notes come with Grow, which has a 14-day trial.

Roster each shift once. Pay and claims follow.
